29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Location: Randy's Carwash - 277 Middlesex Avenue, Medford, MA  The employer did not develop and utilize energy control procedures to protect employees from the unexpected start up and amputation hazards while performing cleaning and maintenance to the salt spreader machines.    On 12/8/11, an employee cleaning the Economizer salt spreader machine on the back of the 2004 GMC 3500 pickup truck while the salt spreader conveyor was powered on, lost the tip of the left thumb up to the first knuckle when the tip of the wash gun and the employees thumb was pulled into the in-going conveyor and caught between the conveyor and the safety bar of the salt spreader.
