29 CFR 1910.212(a)(5):     Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:    On or about February 23, 2012, and at times prior thereto, adequate guarding was not in place on the outdoor fan used for ventilation.  One side of the fans guarding had been loosened to conduct repairs on the fan but was not closed once the repairs were complete.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the fan guarding is in place.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    A) On or about February 23, and at times prior thereto, the switch to operate the outdoor box fan is not enclosed exposing employees to live wiring.    B)  On or about February 23, and at times prior thereto, a breaker box located in th egranite cuttin groom had an uncovered opening, leaving the box open to the dust, debris and penetration by conductive objects (fingers, screwdriver, other tools).    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the switch box is covered.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    On or about February 23, 2012, and at times prior thereto, the electric supply cord to the box fan did not have a strain relief for the connection of the flexible cord to the plug.  This electrical junction without strain relief exposed employees to arcing effects due to loose electrical connections where cord strain is directly transmitted to the wire.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that strain relief is provided for the electrical cord.
