29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by hardhats:    a)Mendoza Demolition Services, Inc., 2280 Ivanhoe Street, Denver, CO:  The employer did not ensure hardhats were worn while an employee worked inside an excavation. Condition exposed an employee to potential head injuries associated with material falling into the excavation.    Abatement Note:  Abatement certification is not required for this item.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).     a)  Mendoza Demolition Services, Inc., 2280 Ivanhoe Street, Denver, CO: The employer did not ensure that each employee working in an excavation over 5 feet deep was protected from cave-in.  On or about February 17, 2012, an employee was observed working outside the protective trench box used to prevent cave in.  The employee left the boundaries of the protective system while tying in a new plastic sewer pipe.       Abatement Note:  Abatement certification is not required for this item.
