29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Employees stand on the four inch lip of plating tanks in order to add nickel to the nickel tanks and to make repairs or adjustments as may be required, such as reconnecting the air hose. The lip of the tank is 23 inches above the platform and the tanks are 60 inches deep and 48 inches across. No documentation or certification required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The point of operation on the #43 Sciaky Welder was not adequately guarded as there was a 3/4 inch opening between the part and the welding tip.
