29 CFR 1910.147(c)(4)(ii):  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules,and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to Items A through D of this section:    (a)  The machine specific energy control procedures for locking out the Kaneda Mechanical Power Press and the Alva Mechanical Power Press were incomplete, in that the procedures did not include guidelines on isolating the stored energy of the rams.      (b)  There were no machine specific energy control procedures for locking out the pullers and cutters.

29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips andsparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.:    (a)  Employees operating the DoAll Horizontal Band Saw, were exposed to 14 inches of unguarded blade.    (b)  Employees operating the MSC Vertical Band Saw, were exposed to 1-1/2 inches of unguarded blade.    (c)  Employees operating the Enco Machine lathe, were expoed to the unguarded chuck.

29 CFR 1910.217(c)(2)(i)(b):  It shall conform to the maximum permissible openings of Table O-10:    (a)  The guard on the Alva Allen Mechanical Power Press did not conform to Table O-10, in that the opening was 3-1/2 inches by 6-1/4 inches and 8-1/2 inches from the point of operation.    (b)  The guard on the Kaneda Mechanical Power Press did not conform to Table O-10, in that the opening was 7-3/4 inches by 5-3/4 inches and 6-1/2 inches from the point of operation.

29 CFR 1910.217(e)(1)(i):  It shall be the responsibility of the employer to establish and follow a program of periodic and regular inspections of his power presses to ensure that all their parts, auxiliary equipment, and safeguards are in a safe operating condition and adjustment. The employer shall maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected:    (a)  There were no annual inspections of the mechanical power presses.
