29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On and before August 19, 2025, the employer failed to ensure that the ingoing nip points, where employees were hand loading material into the EDS Puller Machine on line 12 in the production area, were adequately guarded in that employees could reach the rotating belts, exposing employees to caught in hazards.

29 CFR 1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. a) On or about August 19, 2025, the employer failed to complete a written certification of the workplace hazard assessment for jobs including, but not limited to, extrusion technicians for the EDS Puller Machine and operators in the production area.
