29 CFR 1910.106(d)(4)(iv): An inside storage room was not provided with either a gravity or a mechanical exhaust ventilation system that was designed to provide for a complete change of air within the room at least six times per hour:    a) In Room 60 - On or about 3/7/12, the exhaust ventilation for the flammable storage room was capped in the penthouse.    b) In Room 80 - On or about 3/7/12, the exhaust ventilation for the flammable storage room was non-functional.

29 CFR 1910.1450(e)(3)(iii): The employer's Chemical Hygiene Plan did not include a requirement that fume hoods and other protective equipment functioned properly and/or did not include specific measures that would be taken to ensure proper and adequate performance of such equipment:    a) In Room 71, Lab hood 8451-71B - On or about 3/7/12, the laboratory hood did not have an average face velocity of at least 80 fpm.     b) In Room 67- On or about 3/7/12, room air was not balanced so as to prevent attic air re-entrainment.    c) On the Roof, - On or about 6/21/12, the discharge stacks exhaust number 408-72 and 408-73 were not at least 10 feet above the highest roof level.      d) At Bldg. 8451 - On or about 3/7/12, an air balancing study was not performed following installation of new air handling units to ensure that the hoods and the ventilation system were functioning correctly.
