29 CFR 1926.501(b)(13) - Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    (a) On or about 02/27/2012 at 47 Sayreville Boulevard, Sayreville, NJ 08871: Employees working from a 10/12 pitched roof with eave heights of 11' and 20' were not protected from falls while engaged in residential roofing activities.

(a) On or about 02/27/2012 at 47 Sayreville Boulevard, Sayreville, NJ 08871: The ladders provided were not secured and did not extend 3' above the working surface of the roof to allow for safe access on and off the roof.
