29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: a/ Duro Bag Manufacturing Co./Elizabeth NJ Site - Production Areas: The employer did not ensure that guard rails provided to protect employees from falling extended to all exposed sides. Including, but not limited to: 1) Production Line #3: Platform, located approx. 50 inches from the floor, which is accessed by employees on regular basis, was provided with standard railing and toeboard that had an opening of approx. 19", exposing employees to a fall hazard. 2) Production Line #6: Platform, located approx. 72 inches from the floor, which is accessed by employees on regular basis, was provided with standard railing and toeboard that had an opening of approx. 19", exposing employees to a fall hazard. Condition observed on/or about 2/28/12.

Equipment to include but not limited to: 1) Production Line #3: Shafts rotating approx. 36 inches from ground floor and creating ingoing nip points were not guarded. 2) Production Line #6: Shafts rotating approx. 36 inches from ground floor and creating ingoing nip points were not guarded. 3) Production Line #6: Guard provided for printing rollers was inadequate, in that, it was not secured and can be moved by hand. 4) Production Line #9B: Rollers rotating approx. 33" from ground floor and creating ingoing nip points were not guarded. 5) Production Line #11B: Rotating drum located approx. 20" from ground floor was not guarded. Condition Observed on/or about 2/28/12.
