Ingoing nip point and caught-in hazard was created by employees guiding bags into the bundling processor.    Condition occurred on or about January 28, 2014.     Duro Bag Manufacturing Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.212(a)(1)), which was contained in OSHA inspection number 280876, citation number 1, item number 3 and was affirmed as a final order on August 8, 2012, with respect to a workplace located at 750 Dowd Avenue, Elizabeth, NJ 07201.
