29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are barrier guards, two-hand tripping devices,electronic safety devices, etc.      (a) Employees in the Bedspread Department were exposed to possible entanglement with rotating parts in that the rotating spoked wheels on the ends of several quilting machines were not guarded against employee contact.

29 CFR 1910.219(e)(3)(i):  Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section:      (a) Employees in the Binding Department working with and around the slitting machines were exposed to possible entanglement and amputation hazards from numerous unguarded belt and pulley assemblies on the machines.  The assemblies were open and exposed to employee contact.

29 CFR 1910.305(b)(1)(i):  Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed:      (a) Employees working on and in proximity to a slitting machine in the Binding Department were exposed to potential electrical shock hazards due to a section of conduit having separated from an electrical box leaving the internal conductors exposed to cuts and abrasions from the unprotected metal edges of the electrical box.

29 CFR 1910.305(g)(1)(iv)(A):  Flexible cords and cables were used as a substitute for the fixed wiring of a structure:      (a)  On or about March 8th and 9th of 2012, employees were exposed to potential electrical shock and fire hazards due to a number of instances where temporary flexible cords were used in lieu of permanent building wiring.  This condition was noted to exist in various areas throughout the facility including, but not limited to, the "Retail" area in the basement of the facility.

29 CFR 1910.305(g)(2)(ii):  Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced:      (a) Employees working in the Retail Cutting area of the facility were exposed to electrical shock and fire hazards due to temporary flexible (extension) cords having been spliced directly to the flexible cords of overhead light fixtures.
