29 CFR 1910.37(b)(5): Doorways along an exit access route, that could be mistaken as an exit, were not marked "Not An Exit" or similar designation, or be indentified by a sign indicating it's actual use:    NEX Warehouse, Receiving Dock 1: On March 9, 2012,and at times prior thereto, a door which could of been confused as an exit was not labeled with a designation indicating it's use.

29 CFR 1910.178(a)(5): Where powered industrial trucks were equipped with front-end attachments other than factory installed attachments, the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered:    8th Street Will Call: On March 9, 2012,and at times prior thereto, where six foot extensions for the forklifts were used, the forklifts did not have a name plate to indicate the truck's lifting capacity.
