(b) On March 14, 2012 employees were exposed to open sided floors at door openings that were not guarded on the 2nd and 3rd floor. (c) On March 27, 2012 employees were exposed to open sided floors at door openings that were not guarded on the 2nd floor, 3rd floor, and 4th floor, and other open sided areas on the 2nd floor. (d) On March 28, 2012 employees were exposed to open sided floors at door openings that were not guarded on the 2nd floor, 3rd floor, and 4th floor.
