On or about 03/15/2012, Marine Express Inc., Mechanic Shop area ,Isla Grande Oeste-Panamerican Dock, San Juan PR:  A hustler truck was left unattended with its engine running and not secured against movement.    NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

Employees were operating Hustler Trucks - Capacity #3 without an audible horn / back-up alarm system. 29 CFR 1917.111(a): The structural integrity of docks, wharves, piers, terminals and/or working surfaces was not maintained.    On or about 03/15/2012, Marine Express Inc, Isla Grande Oeste-Panamerican Dock, Yard area, San Juan PR.  Employees engaged in the stevedoring operations were exposed to struck by, caught by and fall hazards as evidenced by the by the terrain conditions on several areas at the marine terminal. Several holes and unleveled terrain were observed where hustler trucks (brutes), with or without trailers, were moving while performing stevedoring operations.    Note:  Certification and Documentation of abatement must be submitted for this citations item as required by 29 CFR 1903.19(c) and (d), respectively
