29 CFR 1910.1200(h)(1):  Employees were not provided with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets. 29 CFR 1926.451(e)(1): When scaffold platforms more than 2 feet (0.6 m) above or below a point of access were not provided with portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.     JOBSITE:  The two (2) employees, engaged in laying concrete block for the chimney of a house, were not provided with a safe means of access while climbing up and down the frames of the scaffold.
