29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: 457 Field Point Road: On or about April 29, 2025, workers were exposed to falling soil and engulfment hazards while working at the bottom of an approximately 12 foot tall excavation face while performing tasks such as (but not limited to) installing a water pump and marking the ground.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. 457 Field Point Road: On or about April 29, 2025, an inspection of the excavation was not performed by a competent person to recognize the unprotected excavation face can expose workers to falling soil and engulfment hazards while working at the bottom of an approximately 12 foot tall excavation.
