29 CFR 1910.23(a)(3): Every hatchway and chute floor opening shall be guarded. a) Global Chemical Resources located on 1925 Nebraska Avenue in Toledo, OH: On or about March 14, 2012, the employer did not assure the filling chute on the gravity flow mezzanine was covered when it was not in use. The uncovered chute opening was approximately 12 inches or more and exposed employees to a fall that was approximately four feet to the bottom of the chute.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section): a) Global Chemical Resources located on 1925 Nebraska Avenue in Toledo, OH: On or about March 14, 2012, the employer did not assure that employees were protected from fall hazards while working on the gravity flow mezzanine. Employees were exposed to a fall from height hazard that was approximately 20 to 25 feet from ground level. All sides of the platform was not equipped with a standard guardrail in accordance with 29 CFR 1910.23(e).

29 CFR 1910.134(e)(1): The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace. a) Global Chemical Resources located on 1925 Nebraska Avenue in Toledo, OH: On or about March 12, 2012, the employer did not assure that employees received a medical evaluation prior to requiring the use of a respirator. Employees were utilizing a 3M full-face cartridge style respirator to protect them from being potentially exposed to organic vapors such as, but not limited to, Formic, Phosphoric , and Acetic acids while performing chemical mixing operations.
