The uncovered chute opening was approximately 12 inches or more and exposed employees to a fall that was approximately four feet to the bottom of the chute.

Employees were utilizing a 3M full-face cartridge style respirator to protect them from being potentially exposed to organic vapors such as, but not limited to, Formic, Phosphoric , and Acetic acids while performing chemical mixing operations.
