29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a) At the worksite, the employer did not ensure that tubular welded scaffold was fully planked while employees performed masory task, thereby exposing employees to an approximate 17 foot fall hazard.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) At the worksite, the employer did not ensure that a railing was provided, other than the crossbracing, to protect employees from an approximate 17 foot fall hazard.
