29 CFR 1910.219(d)(1):     Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    Employees attempting to free lodged balls are exposed to contact with the unguarded portion of the pulley and belts, and the pulley nip points were not suitably enclosed.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    Instance #1  Employees are exposed to burns, cover plate missing on a junction box located on the northwest side of the post between the two Brunswick Automatic Pinsetters.     Instance #2  Employees are exposed to burns, cover plate missing on a junction box located on the northeast side of the post between the two Brunswick Automatic Pinsetters.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.    Employees using an extension cord for the lane cleaner are exposed to electrical shock; the employer failed to inspect the cord visually before use. The cord had cuts to the outer insulation.

29 CFR 1910.334(a)(2)(ii):     If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.    Employees were exposed to electrical shock; employees were using a damaged extension cord to power the lane cleaning machine. The cord had cuts to the outer insulation, the employer failed to remove the cord from service.

29 CFR 1910.1200(e)(1):     Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    Employees using a gallon jug containing diluted Brunswick Invincible Lane Cleaner were exposed to the hazards associated with the substance and the containers were not labeled, tagged, or marked. The employer did not develop and maintain a program that ensured labeling of chemical, maintaining MSDSs for all chemicals being used and training employees using the chemicals.

29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    Employees using Brunswick Authority 22 and Invincible Lane Cleaner were exposed to inhalation hazards, the employer did not provided a copy of a safety data sheet for the chemicals nor were the employees aware of what PPE would be required to use the chemicals.
