29 CFR 1910.1001(j)(2)(i):     The building owner or facility owner did not determine the presence, location, and quantity of presumed asbestos-containing material (PACM) and/or other asbestos-containing material (ACM) at the work site:    a) Worksite - The employer did not identify the presence, location, and quantity of asbestos containing material in areas throughout the facility such as but not limited the ice plant, catwalk, and fire pump room; on or about 3/21/12.     Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1001(j)(3)(v):     The building or facility owner did not post, at the entrance to a mechanical room or area in which employees reasonably could be expected to enter and which contained ACM and/or PACM, a sign which identified the material which is present, its location and the appropriate work practices:    a) Ice Plant Room - Employees step on and work around the evaporator which is covered in insulation that contains asbestos.   Warning signs were not posted at the entrance to the room; on or about 3/21/12.    b) Room at the end of the Underground Tunnel Adjacent to Exhibition Hall - Electricians enter room to perform work such as running temporary wiring over pipes that are insulated with materials that contain asbestos. Warning signs were not posted at the entrance to the room; on or about 3/21/12.    c) Catwalk Truss area - workers work on catwalk where they may inadvertantly come in contact with cords from Leko lights.  Leko light cords were covered with asbestos containing material.  Warning signs were not posted at the entrance to the area; on or about 3/21/12.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1001(j)(7)(iv):     The employer did not provide, at no cost to employee(s) who perform housekeeping operations in an area that contains ACM or PACM, an asbestos awareness training course, which shall at a minimum contain the following elements  health effects of asbestos, locations of ACM and PACM in the building/facility, recognition of ACM and PACM damage and deterioration, requirements in this standard relating to housekeeping, and proper response to fiber release episodes, to all employee(s) who perform housekeeping work in areas where ACM and/or PACM was present, at least annually for each employee    a) Worksite - Custodial workers clean up floors and equipment surfaces throughout the facility in areas that have been identified as having asbestos containing materials. The employer did not provide an asbestos awareness course; on or about 3/21/12.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
