29 CFR 1910.212(a)(1)  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts:  1)  Bay 2, the Nardini-ND 1540 E lathe used approximately twice/week did not have a chuck guard.  2)  Bay 4, the Clausing drill press used approximately once/week did not have a chuck guard.

29 CFR 1910.215(a)(4)  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  1) Bay 1, the Dayton pedestal grinder #862 (3450 rpm) used approximately a couple times/week had the work rest adjusted to the periphery of the abrasive wheel on the left side with an opening which measured approximately 1 inch.  The work rest was adjusted to the periphery of the abrasive wheel on the right side with an opening which measured approximately 7/8 inch.   2) Bay 4, the Dayton 8" bench grinder used for stone grinding daily had the work rest adjusted to the periphery of the abrasive wheel on the right side with an opening which measured approximately 1 inch.

29 CFR 1910.215(b)(9)  A guard for abrasive wheel machine(s)  where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel.   The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:   1) Bay 1, the Dayton pedestal grinder #862 (3450 rpm) used approximately a couple times/week did not have a tongue guard on either the left or right side of the machine and had an opening which measured approximately 1 1/4 inches on the right side and an opening which measured approximately 1 inch on the left side.  2) Bay 4, the Dayton 8" bench grinder used for stone grinding daily lacked a tongue guard on the right side of the machine and had an opening which measured approximately 1 inch.

29 CFR 1910.242(b)  Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  1)  Bay 1, the air nozzle used daily by the operator of the Makino a 81 HMC (horizontal machining center) measured approximately 70 p.s.i.  2) Bay 1, the air nozzle used daily by the operator of the OKUMA (ACE Center MB-56 VA) VMC #616 (vertical machining center) measured approximately 85 p.s.i.  3) Bay 2, the air nozzle used daily by the operator of the Agie Charmille EDM machine #911 measured approximately 100 p.s.i.  4) Bay 2, the air nozzle used daily by the operator of the OKUMA MC-V4020 VMC #619 measured approximately 90 p.s.i.   5) Bay 2, the air nozzle used daily by the operator of the MAZAK INTEGREX 300 IV MILL TURN machine measured approximately 70 p.s.i.  6) Bay 2, the air nozzle used daily by the operator of the OKUMA lathe (Cadet-L1420) #710 measured approximately 85 p.s.i.
