29 CFR 1910.110(d)(10): Employer did not take precautions to prevent damage to LP gas system(s) from vehicular traffic. (a) At the Tonopah field office, inside the yard enclosed by a fence; a 1000 gallon LPG tank was in the driveway used to access the storage building for emergency and maintenance supplies.

29 CFR 1910.141(a)(5): The workplace was not constructed, equipped, and maintained to prevent the entrance or harborage of rodents, insects, or other vermin. A continuing and effective extermination program was not instituted where their presence is detected. (a) At the Tonopah field office on top of employee and supervisors desks, cabinets and floors rodent droppings were observed. Rodent infestation exposed employees to potential hantavirus and other diseases. (b) In the main building computer room and Uninterrupted Power Supply (UPS) room there was evidence of a live moth infestation in ceiling light fixtures, shelf tops, and floors. Moth infestation could result in allergic reactions and/or respiratory ailments.
