29 CFR 1926.501(b)(4)(i):     Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    On March 22, 20112 employees were not protected from a fall as they worked around and installed new skylights, exposing them to a fall hazard of more than six feet.     There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    On March 22, 2012 employees were not protected from a fall as they installed new skylights and ice and water shield on a residential property, exposing them to a fall hazard of 17.9 feet above grade.    There is no abatement certification or documentation required for this item.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      On or about March 22, 2012 employees were required to work from the roof of a residential property and were not trained to recognize or minimize the hazards associated with a fall.    In accordance with 29 CFR 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
