29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On February 9, 2015 employees were not protected from a fall while roofing a commercial property with a low sloped roof, exposing employees to a fall of 16 feet.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:   On February 9, 2015 employees were exposed to fall hazards due to using a ladder that did not extend three feet past the landing they were accessing.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:   On February 9, 2015 a six foot "A" frame ladder was used in the closed position to access an upper landing, exposing employees to fall hazards while using a ladder for purposes other than the purpose for which it was designed.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about February 9, 2015 employees were not provided with training to recognize and minimize the hazards associated with ladder use.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
