Located at 217 Grand Street, Brooklyn, NY    On or about 3/22/2012    a)  Workers using a 6 frame high tubular welded frame scaffold stairway and each level of the stairway was missing an adequate handhold for employees grasping the railing creating a projection hazard.

Located at 217 Grand Street, Brooklyn, NY    On or about 3/22/2012    a)  Workers using a tubular welded frame scaffold stairway and the steps were not flat and vertical with each other on the 1st and second level.

Located at 217 Grand Street, Brooklyn, NY    On or about 3/22/2012    a)  Workers worked from and accessed the roof area using a tubular welded frame scaffold which was not inspected by a competent person during its erection.  The stairway and the steps were not flat and vertical, was missing handrails, and was missing midrails and endrails.

29 CFR 1926.451(g)(4)(xv):  Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart.    Located at 217 Grand Street, Brooklyn, NY    On or about 3/22/2012    a)  Employee worked from and accessed the roof area using a tubular welded frame scaffold, and crossbracing used in place of a toprail was not between 38 and 48 inches .  Employees exposed to a fall of approximatley 30 ft.

Located at 217 Grand Street, Brooklyn, NY    On or about 3/22/2012    a)  Employee on the edge of a building roof and the employee was not protected from falling approximately 40 feet.  Employee was located on the northwest corner of the building and was not protected from falling with a personal fall arrest system.
