29 CFR 1910.134(f): The employee was required to use a respirator without being fit tested with the same make, model, style and size of respirator that was to be used:        (a)  Gambol Industries, boat dock:  A diver was inspecting the underwater hull of the vessel NORTH STAR using SCUBA but was fit tested for use of 3M 7502 dual cartridge respirator.        (b)  Gambol Industries, floating dry-dock:  A diver was working inside the dry-dock centering the vessel NORTH STAR using SCUBA and was not fit tested for use of the equipment.        c) Gambol Industries, floating dry-dock: The dive team supervisor was assigned as standby diver requiring the use of SCBA was fit tested for use of 3M 6300 dual cartridge respirator.

(a) Gambol Industries :  The employer allowed employee to conduct diving operations and did not make a copy of the safe practices maual available to each dive team member at the dive location.  On April 20, 2012 a dive was performed inside the floating dry-dock during a centering operation and the diver stated he had never seen the Safe Practices Manual.
