29 CFR 1926.405(g)(2)(iii): Flexible cords were not used only in continuous lengths without splice or tap. Splices on No. 12 or larger flexible cords that had been repaired did not retain the insulation, outer sheath properties, and usage characteristics of the cord being spliced. On or about March 6, 2012, and times prior thereto, flexible cords were not used in continuous lengths without splice of tap. Repairs of spliced cords did not retain original outer sheath properties and usage characteristics. Cords were used to power portable tools such as, but not limited to, circular saws and drills. Employees were exposed to the hazard of electric shock.

Employees were exposed to the hazard of falls. ANTONIO MONTOYA FRAMING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316033836, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 12, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 5050 N. Maize Rd., Maize, KS 67101.
