Periodic inspections of the company's specific energy control procedures were inadequate in that they did not include a review with each authorized employee of the specific energy control procedures developed by the company.

Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.    Employees who perform energized electrical work on 110 Volts AC to 480 Volts AC consisting of troubleshooting, voltage testing involving the use of a multi-meter and amperes using an amp probe had not received training on what and when to use personal protective equipment to protect themselves against electric shock and arc flash.

29 CFR 1910.335(a)(1)(i):     Employees working in areas where there are potential electrical hazards shall be provided with,and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.Note: Personal protective equipment requirements are contained in subpart I of this part. 29 CFR 1910.212(a)(3)(ii):     The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    On or about March 12, 2012, in the Rework Area, an employee was exposed to a amputation hazard while using a Cold Saw Machine with an inadequate point of operation guard to saw 1/2 inch square metal tubing bundles.    Middletown Tube Works, Inc. was previously cited for a violation of the occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 314100280, citation 1, item 1, which was affirmed as a final order on March 8, 2010, with respect to a workplace located at 2201 Trine Street, Middletown, OH 45044

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    In the Maintenance Area, an employee was exposed to a electric shock hazard from the flexible cord on the Miller Portable Arc Welding uint pulled away from the fitting on the 220 Volt AC electrical disconnect switchbox, creating strain on the terminal screws.    Middletown Tube Works, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 314100280, citation 1, item 5, which was affirmed as a final order on March 8,2010, with respect to a workplace located at 2201 Trine Street, Middletown, OH 45044.
