29 CFR 1910.95(c)(1):  A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8 hour time-weighted average sound level (TWA) of 85 dBA:    a)  An employee working as a Sprial Operator was exposed to continuous noise at a sound level of approximately 87.3 dBA (8 hour time weighted average) during the 480 minute sampling period on 4/12/12.  Employee was wearing ear plugs.    b)  An employee working as a Mechanic was exposed to continuous noise at a sound level of approximately 89.4 dBA (8 hour time weighted average) during the 480 minute sampling period on 4/12/12.  Employee was wearing ear plugs.    c)  An employee working as a Apprentice was exposed to continuous noise at a sound level of approximately 90.4 dBA (8 hour time weighted average) during the 464 minute sampling period on 4/12/12. Exposure calculations include a zero increment for the 16 minutes not sampled.  Employee was wearing ear plugs.    d)  An employee working as a Mechanic was exposed to continuous noise at a sound level of approximately 89.1 dBA (8 hour time weighted average) during the 467 minute sampling period on 4/12/12. Exposure calculations include a zero increment for the 13 minutes not sampled.  Employee was wearing ear plugs.    e)  An employee working as a Apprentice was exposed to continuous noise at a sound level of approximately 92.1 dBA (8 hour time weighted average) during the 447 minute sampling period on 4/12/12. Exposure calculations include a zero increment for the 33 minutes not sampled.  Employee was wearing ear plugs.    f)  An employee working as a Utility Man was exposed to continuous noise at a sound level of approximately 93.4 dBA (8 hour time weighted average) during the 454 minute sampling period on 4/12/12. Exposure calculations include a zero increment for the 26 minutes not sampled.  Employee was wearing ear plugs.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:    a)  Throughout the facility, on or about 4/12/12:  No periodic inspections were conducted of the energy control procedures to ensure that the procedures and requirements of the standard were being followed.  Employees perform servicing and/or maintenance on various machinery or equipment including but not limited to:  Cybermation Machines; Sprial Machines; Coil Line Machines and Turning Machines.

29 CFR 1910.157(g)(2):  The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    a)  Throughout the facility, on or about 4/12/12: Training was not provided annually to employees on the use of fire extinguishers and the hazards involved with incipient stage fire fighting.    * ABATEMENT NOTE:  By this date the employer must either correct the alleged violation or implement a Fire Safety Policy; as outlined in 29 CFR 1910.38(a) and .39 which includes the evacuation requirements of 29 CFR 1910.157(b).
