OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee(s) were exposed to a crushing hazard:    (a)  At the facility employee(s) working / walking along the Packing Bay in warehouse 1 where steel storage racks had not been periodically inspected for corrosion, structural damage, or overloading.  Column leg 2 on the west wall was severely bent with pallets of materials still in the storage racks, thus exposing employee(s) to physical injuries.    (b)  At the facility employee(s) working / walking along the Packing Bay in warehouse 1 where steel storage racks had not been periodically inspected for corrosion, structural damage, or overloading.  Column leg 3 on the east wall was severely bent with pallets of materials still in the storage racks, thus exposing employee(s) to physical injuries.    (c)  At the facility employee(s) working / walking along the Packing Bay in warehouse 1 where steel storage racks had not been periodically inspected for corrosion, structural damage, or overloading.  Column leg 7 on the east wall had the bracing support severely bent with pallets of materials still in the storage racks, thus exposing employee(s) to physical injuries.    (d)  At the facility employee(s) working / walking along in warehouse 2, row 1, column leg 6 (rack identification number - 20119) where steel storage racks had not been periodically inspected for corrosion, structural damage, or overloading.  Column leg 6 was severely bent with pallets of materials still in the storage racks, thus exposing employee(s) to physical injuries.    (e)  At the facility employee(s) working / walking along in warehouse 2, row 2, column leg 6 (rack identification number - 1545) where steel storage racks had not been periodically inspected for corrosion, structural damage, or overloading.  Column leg 6 was severely bent with pallets of materials still in the storage racks, thus exposing employee(s) to physical injuries.    Among other methods, one feasible and acceptable method to correct this hazard is:  Develop and implement a structural integrity program to periodically inspect storage racks as recommended by the Rack Manufacturing Instutite Specification for the Design, Testing, and Utilization of Industrial Steel Storage Racks 1997 Edition, section 1.4.1.    29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violations have been completed.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    (a)  Employee(s) working / walking around a column post north of dock doors 8 & 9 in warehouse 2 were exposed to electrical hazards due to a damaged receptacle box exposing live conductors.    (b)  Employee(s) working / walking around the south wall of record storage in warehouse 2 were exposed to electrical hazards due to 1-1/2 inch of live wiring exiting from conduit, not being guarded.    (c)  Employee(s) operating the Campbell Hausfeld compressor were exposed to electrical hazards due to 6 - 7 inches of the flexible conduit unraveling, thus exposing live conductors.     29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
