29 CFR 1910.134(e)(1):  The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace:  (a)  Blazer Waterproofing Systems Inc dba Blazer Structures at 931 14th Street, Denver, CO 80202:  On and before March 14, 2012, the employer did not provide a medical evaluation to each employee wearing a half-mask elastomeric respirator as part of their job duties.  Employees wore half-mask elastomeric respirators during application of isocyanate-containing coating materials to the floor of the first floor mechanical room.  This condition exposes employees to the physiological hazards of wearing a respirator without the evaluation of a physician or other licensed healthcare professional.    Abatement Note:  Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

Employees applied a three-layer coating to the floor.  The coatings contain chemicals such as, but not limited to, epoxy, isocyanate, and polyurethane.
