29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: a) Blazer Waterproofing Systems Inc located at 1022 Eagles Nest Circle, Vail, CO 81657: On and preceding 9/4/19, the employer training did not include the measures employees could take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used. An employee was installing waterproofing insulation with a spray adhesive in a 7 foot concrete window well of a multi-family residential foundation with no means of egress and without ventilation equipment. A flash fire occurred due to an unknown ignition source. The employee had to self-extricate out of the window well. This condition exposed the employee to chemical and thermal hazards.

This condition exposed the employee to thermal and trapped in hazards.
