29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      JOB SITE: 401 Gaylord Drive, Fulton, MO 65251, at this work site, workers were observed using a Louisville aluminum extension ladder, Model L-2222-24, to gain access onto the roof and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately remove from service:      JOB SITE: 401 Gaylord Drive, Fulton, MO 65251, at this work site, workers were observed using a Louisville Aluminum extension ladder, Model L2222-24, to gain access onto the roof, approximately 18 feet 5 inches, with the ladder feet broken off or removed.  The workers were exposed to a fall hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.        JOB SITE: 401 Gaylord Drive, Fulton, MO 65251, at this work site, a workers was observed standing on the edge of the parapet of a commercial building, installing roof extensions, approximately 18 feet 5 inches above next lower level, and was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety monitoring system.  The worker was exposed to a fall hazard.      Glovecon was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 314464082, citation number 1, item 1, issued on 7/15/2010 and was affirmed as a final order date of 8/09/2010, with respect to a workplace located on 221 West High Street, Jefferson City, MO 65102.
