29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective. On or about February 28, 2012, at Collin D#4 Well , Midland TX, Stallion Oil Field Services Inc., employer did not ground properly the sewer tank to the crew house. The sewage tank ground rod was barried 4 inches into the ground. This conditions did not allow a pemanent and cotinuous path to ground from circuits, equipment and enclosures. Employees were exposed to potential electrical hazards.
