29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees: On or about February 13, 2012, the employer did not ensure that a natural gas pipeline was protected, supported or removed while the excavation was open. The excavation collapsed and caused the pipeline to explode. Employees were exposed to explosion hazard, natural gas exposure and potential fire hazards.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: On or about February 13, 2012, a competent person did not conduct a daily inspection of the excavation to identify and recognize potential or existing cave-in hazards. The excavation collapsed and employees were exposed to a gas line explosion, gas fume overexposure and potential fire hazards.
