29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.     At the establishment, employees were not provided fall protection while performing welding operations on the tops/roofs of Explosion Resistant Enclosures. Employees were exposed to a fall hazards of approximately eight (8) feet to the concrete floor below.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    At the establishment, the employer did not establish an energy control program. Employees are exposed to amputation, struck-by and caught between hazards.

29 CFR 1910.178(l)(2)(ii): Powered Industrial Truck Operator Training. Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    At the establishment the employer did not perform powered industrial operator training. Employees were exposed to  struck by hazards.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.    At the establishment, the employer did not require examine forklifts before being placed into service. Employees were exposed to struck-by hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.    At the establishment flexible cord sets and cord connected equipment listed below were not visually checked prior to use. Employees were exposed to elelctrcal shock hazards.    1. Finish Shop, a portable light was being utilized that had damaged insulation.  2. Fab Shop, an orange extension cord was plugged in and available for use that had damaged insulation.  3. Finish Shop, two extension cords were in use with missing gound pins.

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.    The employer did not utilize the OSHA 300, 300-A and the 301 forms or equivalent forms to record work related injuries as required.
