29 CFR 1910.147(c)(6)(ii): The annual certification of the energy control procedures did not identify the machine or equipment on which the energy control procedure was being utilized:  The 2013 and 2014 annual certification of the energy control procedures indicated that the audit was performed, however, it did not indicate which machines/equipment the annual audit was performed on. Employees were exposed to struck-by and/or shock hazards.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  The employer has not updated the hazard communication program and trained employees on the portion of the hazard communication program that explains the labels on shipping containers and the safety data sheets for items such as but not limited to fuel gases, lubricants and paint, including the order of information and how the employee could obtain and use the hazard information.
