29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: a) On or about March 29, 2012, and at times prior, the employer did not ensure its employees performing line breaking at the Xcel Pueblo Comanche Power Station did not verify that all hazardous energy sources associated with the Air Handler Demolition Project were deenergized and isolated.
