Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers shall conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition), which are incorporated by reference in & sect; 1926.200(g)(2).          a)     At the worksite, the employer did not ensure that flaggers were posted along the east and west ends of the county road, to control traffic around and/or past the east bound lane that was blocked with equipment and/or material, thereby exposing employees to potential struck-by hazard.

29 CFR 1926.202:     Barricades for protection of employees shall conform to Part VI of the Manual on Uniform Traffic Control Devices (1988 Edition, Revision 3 or Millennium Edition), which are incorporated by reference in 1926.200(g)(2). [67 FR 57736, Sept. 12, 2002]          a)     At the worksite, the employer did not ensure that barricading such as, but not limited to cones, were adequately placed and angled for employees protection against east and west bound traffic along the county road, where the east bound lane was close, sections at a time, thereby exposing employees to a potential struck-by hazard.

A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.          a)     At the worksite, the employer did not ensure that employees had safe access/egress, in and/or out of trench, thereby exposing employees to a potential caught-in hazard.
