29 CFR 1926.251(a)(1):     Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service.    a. At the excavation site located at 812 Georgres Creek Rd. in Gallipolis, Ohio, a four part 3/4" wire rope bridal was used as rigging to lift trench boxes in and out of a 12' trench.  There were no safety latches on all four hooks of the rigging, thereby exposing the employees to struck-by and caught-between hazards.

29 CFR 1926.251(a)(2)(i):     Employers must ensure that rigging equipment:  Has permanently affixed and legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load;    a. At the excavation site located at 812 Georgres Creek Rd. in Gallipolis, Ohio,  a four part 3/4" wire rope bridal was used to lift trench shields in and out of a trench.  The rigging did not have any markings to indicate the safe working load, thereby exposing the employees to struck-by and caught-between hazards.

29 CFR 1926.652(a)(1)(ii):     Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  Excavations are less than 5 feet (1.52 m) in depth and examination of the ground by a competent person provides no indication of a potential cave-in.    a. At the excavation site located at 812 Georgres Creek Rd. in Gallipolis, Ohio, employees were working at manhole 56-57 on a sewer line in a trench that was 12 feet deep without any protection on one end of a trench sheild that was installed against the end of the excavation, thereby exposing employees to the hazards of a cave-in.

29 CFR 1926.652(g)(1)(iii):     Employees shall be protected from the hazard of cave-ins when entering or exiting the areas protected by shields.    a. At the excavation site located at 812 Georgres Creek Rd. in Gallipolis, Ohio, employees were entering and exiting at the end of a trench that was 12 feet deep without protection if the end of the the trench were to collaspe, thereby exposing the employees to cave-in hazards.
