29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    The employer failed to perform periodic inspections to their energy control procedures had been conducted at least annually to ensure that employees were following employers procedures. Lockout tagout is performed on fabrication equipment such as mechanical power presses, bodymakers, autopackers, waxers, trimmers, etc., during servicing and maintenance operations.     Abatement documentation was provided.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    The employer failed to use and enforce methods of machine guarding that would protect employees against hazards such as ingoing nip points, rotating parts, and flying chips. Employees were exposed to cuts, lacerations, fractures, amputations while operating the following equipment:    Fabrication Area    1) 1785 Bodymaker - Missing front and posterior guard.     Abated 4/16/12.    2) 2082 Autopacker- Relocate interlock, safety switches to prevent employee access while in operation.     Abated 3/30/12.    3) 1046 Trimmer- Blade cover was not fastened or secured to prevent access while in operation.     Abated 4/4/12.    4) 1047 Trimmer- Blade cover was not fastened or secured to prevent access while in operation.     Abated 4/4/12.    5) 2128 Waxer- Inadequate guarding along the belts. Additional guarding required to eliminate nip points.     Abated 4/4/12.    6) 2582 Waxer- Inadequate guarding along the belts. Additional guarding required to eliminate nip points.     Abated 4/4/12.     7) Seamer and turn table 2011- inadequate guarding on conveyors exposing employees to nip points.     Abated 3/30/12.      Abatement documentation was provided.
