29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Duplex slitting machines - hazardous energy control procedures (i.e. lockout tagout) were not utilized for an orderly shutdown of the equipment and isolation of hazardous energy during setup and other unjamming servicing tasks. Employees had their finger(s) and hand(s) exposed to caught in-between hazards capable of causing amputations. Use of the stop button and/or emergency stop device (alternative measures) did not provide effective protection against unexpected start-up.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  Duplex slitting machines - prior to starting work, employees assigned to perform setup and other unjamming servicing tasks were not required to apply energy controls in the following sequence:   (a) turn off and shut down the machines, (b) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (c) affix lockout or tagout devices to each energy isolating device, (d) verify that isolation and deenergization of the machine or equipment have been accomplished.  Employees were exposed to caught-in hazards likely to result in amputation of hand(s) and fingers.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Duplex slitting machines - provided plexiglass swing-type guards did not effectively safeguard all access side openings to the point-of-operation and associated danger zone. Guards were not secured and/or fastened to the machine. Employees were exposed to caught in-between pinch point hazards capable of causing amputation.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  The J. L. Clark, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 328959, citation number 01, item number 003 and was affirmed as a final order on September 25, 2012, with respect to a workplace located at 923 23rd Ave. Rockford, IL 61104.
