Spancrete was previously cited for a violation of this Occupational Safety and Health Standard, or an equivalent Standard, which was contained in OSHA Inspection Number 312558075, Citation #1, Item #1, which was affirmed as a Final Order on 01/17/11, with respect to a workplace located at Golds Gym, Appleton, WI 54914.
