29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Entire Facility. The employer did not have an established lockout/tagout program for employees who performed servicing and maintenance on machines and equipment such as but not limited to saws, CNC routers, perforators, laminator and dust collectors (auger cleanout).     Abatement Note:  A lockout/tagout program should include the following key elements:    - Energy source evaluation  - Development, documentation and utilization of machine-specific procedures  - Procedures for group lockout (when more than one employee is servicing the same equipment at the same time)  - Training for "authorized," "affected," and "other" employees  - Periodic inspections (at least annual)  - Sharing of lockout/tagout program information with outside contractors    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed.    (a) Shipping and Receiving. The Pro-Tech radial chop saw did not have a self-adjusting guard covering the exposed lower portion of the blade.    There is no abatement certification or documentation required for this item.
