29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: (a) Jonas Graber Builders LP on or about April 04, 2012, at the address of 2100 Stirrup Ln. Toledo, OH Building D: The employer failed to ensure employees utilizing an aerial lift to access the roof level were adequately protected from falls. Employees were not tied off to the basket of the aerial lift while the basket was elevated. The employees were exposed to an approximate 17ft fall to the ground below when transitioning from the lift's basket to the roof level.
