29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    A. Jonas Graber Builders LP worksite located at 333 Dustin Rd., Oregon, Ohio: On or about July 16, 2012, the employer did not ensure employees wore proper eye protection while operating a pneumatic nail guns when performing framing activities on the west side of the commercial structure. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    A. Jonas Graber Builders LP worksite located at 333 Dustin Rd., Oregon, Ohio: On or about July 16, 2012, the employer did not ensure employees were protected from fall hazards while installing fascia board on the west side of the commercial structure. The employee was exposed to a fall distance of approximately 11.00 feet to the lowest level while walking/working on the exterior top plate.     B. Jonas Graber Builders LP worksite located at 333 Dustin Rd., Oregon, Ohio: On or about July 16, 2012, the employer did not ensure employees were protected from fall hazards while installing trusses near the southeast corner of the commercial structure. The employee was exposed to a fall distance of approximately 11.00 feet to the lowest level while walking/working on the exterior top plate.     Jonas Graber Builders LP was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501, which was contained in OSHA inspection number 311610232, citation number 1, item number 1 and was affirmed as a final order on September 26, 2009, with respect to a workplace located at Monclova, OH.
