29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardousenergy when employees are engaged in the activities covered by this section: Employees performing maintenance and set-up tasks were exposed to amputation, struck-by and caught-in-between hazards due to the unexpected start-up or energization of machines such as but not limited to, CNC milling machines, lathes, and mills in that lockout/tagout procedures were not developed.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating lathe chucks: Employees operating lathes were exposed to amputation, struck-by and caught-by hazards. The following lathes were not provided with chuck guards: a) Southbend 550 horizontal lathe located between the Mold Area and Polish Area. b) Colchester horizontal lathe located between the Mold Area and Polish Area. c) Crown-4 horizontal lathe located between the Mold Area and Polish Area. d) Webb horizontal lathe located near the CNC area along the north wall.
