29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    (a)  Jobsite, 234 Summerset At Frick, Pittsburgh, PA:  On or about 04/05/12 an employee working on the roof at approximately 20 feet above the ground was not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems.

Note: The following appendices to subpart M of this part serve as non-mandatory guidelines to assist employers in complying with the appropriate requirements of subpart M of this part.[59 FR 40738, Aug. 9, 1994; 60 FR 5131, Jan. 26, 1995]    (a)  Jobsite, 234 Summerset At Frick, Pittsburgh, PA:  On or about 04/05/12 an employee did not retain the requisite understanding in the use of fall protection systems and equipment in that the employee was working at a height of approximately 20 feet above the ground without being tied off exposing the employee to fall hazards.
