29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) Jobsite, Deerfield Ridge, Site 16, Cuddy, PA: On or about 06/09/16 a 40 ft. ladder, extended to 38 ft., could not be extended 3 ft. above the upper landing and a grasping device was not provided to assist the employees in mounting and dismounting the ladder.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite, Deerfield Ridge, Site 16, Cuddy, PA: On or about 06/09/16 an employee engaged in residential construction activities was not prevented from falling approximately 36-38 feet to the ground by personal fall arrest systems, guardrail systems or safety net systems. The employer, Zillweger Contracting Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 330783, citation number 1, item number 1 and was affirmed as a final order on 05/09/2012, with respect to a workplace located at 234 Summerset at Frick, Pittsburgh, PA.
