29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On 4/6/12, the 88 ton Cincinnati brake press P3 in work center 525 was not guarded; the light curtain controls for the press did not stop the press from operating when the foot pedal was activated with the employees hand in the die. The tip of operator's right middle finger was amputated when press brake was cycled with foot control. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED
